總膽汁酸（total bile acid ，TBA）是什症膽固醇在肝髒分解及腸-肝循環中的一組代謝產物 ，是总胆汁酸状膽固醇在肝髒分解代謝的最終產物 ，與膽固醇的什症吸收 、代謝及調節關係密切。总胆汁酸状人體的什症總膽汁酸分為初級膽汁酸和次級膽汁酸兩大類。
初級膽汁酸以膽固醇為原料，总胆汁酸状參與脂肪的消化吸收。其經過膽道係統進入十二指腸後 ，在腸道細菌作用下經水解反應生成次級膽汁酸 。當肝細胞發生病變或肝內外阻塞時 ，膽汁酸代謝發生障礙反流入血 ，血清總膽汁酸濃度升高 。因此 ，總膽汁酸水平變化可敏感地反映肝髒功能 。

血清總膽汁酸是肝實質性損傷及消化係統疾病的一個較為靈敏的診斷指標。總膽汁酸能較為特異地反映肝排泄功能，一旦肝細胞有病變或腸-肝循環障礙 ，均可引起總膽汁酸升高。
血清總膽汁酸增高 ：可見於各種急慢性肝炎 、乙肝攜帶者或酒精性肝炎（TBA對檢出輕度肝病的靈敏度優於其他所有肝功能試驗） ，還可見於絕大部分肝外膽管阻塞和肝內膽汁淤積性疾病 、肝硬化 、阻塞性黃疸等 。

1、總膽汁酸偏高提示肝細胞發生病變 ，血液中膽汁酸含量升高 。急性肝炎 、慢性活動性肝炎 、肝硬化 、肝癌時膽汁酸明顯升高 。特別是肝硬化 、肝癌時總膽汁酸的升高率>（95%），也大於丙氨酸轉氨基酶（ALT）20% 。
2 、當肝髒實質損害時，肝細胞對膽酸合成降低，鵝脫氧膽酸的合成升高。
3 、肝實質細胞損傷時 ，CA/CDCA比值小於1.0 。
